This application provides us with valuable information and outlines fees. Please make sure you provide an Email address, printed clearly, and it is correct for both the student and one parent. NDA Crew Form 5 – (OHSAA) Ohio High School Athletic Association Preparticipation Physical Examination Form
The State of Ohio has expanded the required medical form. Page 1 is completed by the student and her parent(s) before participation in interscholastic athletics. A physician must complete Page 2, sign, and acknowledge clearance to participate or not in the sport.

NDA Crew Form 6 – Toledo Rowing Club Safety Policy and Water/Air Temperature Policy
Reference the Safety and Cold Water Policy. NDA Crew Form 6 must be signed and turned in before team members may practice at the boathouse.

NDA Crew Form 7 - USRowing Waivers
Along with the Toledo Rowing Club (TRC) waiver, each regatta requires an original signed waiver for participation at the event. Complete the entire lower portion of each waiver, beginning with “Printed Name of Participant” for all nine of the waivers stapled in the packet. Also, complete the lower portion of three blank waivers that will travel to each regatta, in case (as has happened in the past) the regatta officials are missing waivers for our team. Please note that TRC requires that a Waiver of Liability be on file before club members are permitted to practice at the boathouse, even if they are not yet on the water. Reference the Regatta Schedule for names of regattas we are attending this season.

Please double check to make sure that all sections are completed and all parties have signed the forms. Incomplete forms will be returned. Do not submit forms until you have received notification at the beginning of each season on the due date.
